    Please eliminate this *Condition* from my chart and leave my Levofloxacin treatment with my sinus report.

    I have allergies and sinus problems year round, so if I'm careful and time it right, then taking a short course of antibiotics prevents the infection from turning into Pneumonia. To me treating a sinus infection for 5 days, hoping to prevent Pneumonia, which was successfully prevented on this occasion isn't treating Pneumonia.

    Also, I deleted the other *Condition*, Migraine, because I have had a long history of headaches and I would like to recover, retain that information. Otherwise, the history of my other headache treatments are left alone and don't correlate with any symptom.

    Yes! I'm recovering from last August's medication mismatch, which caused liver, heart damage,
    edema and six months of treatments by a wound care specialist, to heal an internal foot injury
    that would not close and heal. The skin produced by the artificial skin-grafts is still no as tough
    as the rest of my foot, so it can't tolerate a shoe for long, or even a tight sock.
